Palliative Care Physician – Atlanta, GA or Jackson, MS
We’re looking for a mission-driven Lead Palliative Care Physician to join our innovative, patient-first organization transforming kidney care across the U.S. In this role, you’ll be a community specialist serving patients with chronic kidney disease, end stage renal disease, and other complex, polychronic conditions —with an emphasis on quality of life, compassion, and coordinated care.
You’ll work closely with a collaborative team of providers and a Market Medical Director, delivering care primarily via telehealth and in patients’ homes . This is your chance to help redefine palliative medicine while maintaining the time and flexibility to truly connect with your patients.

What You’ll Do:
Serve as the lead physician for palliative care within your market
Conduct advance care planning and meaningful goals-of-care discussions
Manage medication panels for patients with complex needs
Partner with interdisciplinary teams to provide seamless, compassionate care
Maintain privileges at a local hospital; hospice connections a plus
What We’re Looking For:
MD or DO, Board Certified in Hospice and Palliative Medicine
Skilled in advance care planning and comfortable managing complex medication regimens
Experience with Medicare Advantage, managed Medicaid, and commercial patient populations
Active, unrestricted medical license and free of Medicare/Medicaid sanctions (willingness to obtain multi-state licensure as needed)
Reliable transportation for in-home patient visits
Open to current Hospice & Palliative Medicine fellows
