Hybrid RN Case Manager
Looking for a Hybrid RN Case Manager , Hybrid Remote and in Field

Hours: Monday – Friday, 8am-5pm, no weekends, no holidays

Pay: $44.14 per hour
Travel in County Required:

• Shift/Hours: M-F 8-5, training hours may vary
• Location: Fully remote (never coming onsite)
• Travel: Up to 75% travel within or face-to-face visits in various settings including homes, hospitals, provider offices
• Environment: Work autonomously with critical thinking and problem-solving skills required
• Travel time is paid at the hourly wage
• Mileage is reimbursed at the IRS rate
• Must live in Counties: Barry, Van Buren, Kalamazoo, Calhoun, Branch, St Joseph, Cass, and Berrien Counties)

Hybrid RN Case Manager Job Duties:

• Assess and evaluate members with potential care management needs through telephonic and face-to-face assessments in various settings including private residences, hospitals, behavioral, and long-term nursing facilities.
• Establish a cost-effective, member-centric care plan in collaboration with members, authorized caregivers, and providers.
• Monitor and evaluate the effectiveness of care plans, adjusting based on clinical judgment and member needs.
• Coordinate and collaborate with members, authorized representatives, primary care providers, and other care team members to ensure timely service delivery.
• Advocate for members' needs to ensure safe discharge post-hospitalization or transition from nursing facilities, addressing social needs such as housing and food insecurity.
• Maintain accurate and timely documentation in the members' electronic health records.

Hybrid RN Case Manager Requirements:

• RN with current unrestricted licensure in the State
• Verified High School Diploma or GED
• Bachelor's degree in Nursing (BSN) preferred
• Certifications such as CCM preferred

Hybrid RN Case Manager Preferred Qualifications:

• Long Term Care experience preferred
• Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
• Effective communication skills, both verbal and written
