Hospice Medical Social Worker – Full-Time & Part-Time Opportunities

Your Primary Responsibilities:

  • Assessment: Evaluate the psychosocial needs of hospice patients and families, communicating findings to the interdisciplinary care team.

  • Counseling: Provide emotional support, crisis intervention, and guidance to help patients and families cope effectively.

  • Education: Assist with advance directives, patient rights, and connection to community and financial resources.

  • Collaboration: Partner with local agencies and healthcare providers to coordinate holistic care.

  • Participation: Engage in interdisciplinary team meetings and contribute to individualized care plans addressing psychosocial elements of care.

Qualifications:

  • Education:

    • Master’s Degree in Social Work (MSW) from an accredited program

  • Experience: Minimum of one year in healthcare; hospice or home health experience preferred but not required.

  • Skills: Strong communication, organization, and deep understanding of hospice and end-of-life care philosophy.

  • License: Current Arizona state licensure, valid driver’s license, and reliable, insured vehicle.
